Heads roll in US over nuke warheads on B-52

According to Bloomberg, " three senior officers (of the US Air Force) have been relieved of command for their roles in mistakenly allowing a B-52 bomber to fly from North Dakota to Louisiana carrying armed nuclear warheads" on 30 August. The six nuclear warheads were live at the time of the B-52' s journey from North Dakota to Louisiana.

Nobody in Kerajaan Malaysia ever gets "relieved of their duty" for lapses in carrying out their duty. Certainly not senior figures in the government.

Middle Ring Road misconstruction, Parlimen bocor, PKFZ mega ghost town, Express Bus disaster zone etc. etc. nobody ever takes responsibility and accountability.

exactly. no country is faultless but the point is that in better countries like the US, there are cultures and institutions of public accountability to rectify errors and mistakes.
